Which documents are required?
Taking into account the harmful factors of the work environment, the employer issues a mandatory health check-up card (in 2 copies) to the employee, with which the employee goes to the health care institution.

You must bring all medical documents (if any) (examination results, blood test results, specialists' conclusions, extracts from DEĀK (if you have a disability), etc.).
